Neuralgia del trigémino

La neuralgia del trigémino es un tipo especial de dolor de la cara o dolor facial, de alta intensidad y que requiere de tratamiento especializado, llegando en algunos casos a necesitarse una intervención de neurocirugía para superar los molestos síntomas.

¿Qué es el nervio “Trigémino”?

El nervio Trigémino es uno de los nervios craneales, existiendo uno de estos nervios para cada mitad lateral de la cara. Su función específica es dar la sensibilidad de la mitad de la cara correspondiente, además de ser el nervio de los movimientos de la masticación. Se llama “tri-géminos” porque posee 3 ramas encargadas de estas funciones. Neuralgia del trigémino se refiere a un dolor primario del trigémino (sin causal específica que justifique los síntomas en la cara), o sea, cuando no existe una afección como heridas, alteraciones dentarias y otros que normalmente deben producir este dolor.

 

 

¿Por qué se produce la neuralgia del trigémino?

Cuando se han descartado causas normales de dolor del trigémino (de la cara, sobre todo alteraciones dentarias como caries, abscesos, infecciones de la boca o de los huesos de la cara, heridas complicadas y otros), el dolor en general se debe a pequeñas deformidades o  malas posiciones de algunos pequeños vasos sanguíneos (arterias o venas) que pueden tocar este nervio en su paso por la base del cráneo. Estos vasos sanguíneos son normales, o sea, no se trata en general de una malformación o de vasos innecesarios, sólo de posiciones algo diferentes que desencadenan los síntomas ya que con los latidos normales de las arterias el nervio interpreta que se trata de un estímulo doloroso intenso y entonces genera los síntomas de dolor facial. Otras veces, las menos, se encontrarán tumores o infecciones que generan el dolor por contacto directo con el nervio, pero estos casos no se agrupan dentro de la neuralgia primaria del trigémino a la que nos referimos acá.

¿Cómo es el dolor de la neuralgia del trigémino?

Se trata siempre de un dolor a un solo lado de la cara, de tipo punzante, que se puede irradiar hacia el conducto auditivo; que suele desencadenarse por un estímulo normal en la cara (por ejemplo frío o calor, hablar, comer, tocarse determinadas zonas, sonreír, estornudar o toser, etc.). La intensidad del dolor es variable y puede ir desde dolores tolerables, de mediana intensidad, hasta los dolores más intensos que los seres humanos pueden sufrir. Cuando los dolores son muy fuertes es cuando uno se enfrenta al cuadro clásico de la neuralgia del trigémino, ya que aquí el paciente está limitado e invalidado para realizar ciertas actividades básicas con normalidad (comer, hablar, etc.) por el gran temor que tiene a volver a sufrir el dolor.

 

¿Quiénes pueden sufrir de neuralgia del trigémino?

Cualquier persona puede sufrirlo. En general, pero no siempre, se produce después de los 50 años de vida. Cuando no hay factores normales que produzcan el dolor (como problemas dentarios), no hay predisposición a este dolor por otros antecedentes de enfermedades personales o familiares. Es importante recalcar que se debe siempre descartar enfermedades dentales entes de hacer este diagnóstico, ya que los dolores son similares y muchas veces se confunden.

¿Cómo se puede tratar la neuralgia del trigémino?

El primer enfrentamiento es con medidas generales: evitar los estímulos en las zonas que el paciente reconoce que producen el dolor (no tocarse) y una evaluación y tratamiento dental. Se inicia el tratamiento con medicamentos específicos que el especialista indica según la magnitud de las molestias. Estos medicamentos pueden ser antineuríticos como la Carbamazepina, la Gabapentina o la Pregabalina; o estabilizadores de neurotransmisores como la amitriptilina en combinación con otros analgésicos, antineuríticos o relajantes. Las dosis de estos remedios y otras alternativas de tratamiento médico más específico dependen del paciente, de la intensidad del dolor y del tiempo de evolución del dolor. Cuando los tratamientos con remedios no han dado los resultados esperados y el paciente persiste con un dolor importante, se deberá recurrir en muchos casos a la neurocirugía para aplacar el dolor.

¿Cómo es la cirugía de la neuralgia del trigémino?

Existen varias técnicas y enfrentamientos para la cirugía. Hoy en día, las terapias más utilizadas son los procedimientos ablativos o de micro-lesiones del ganglio de donde sale este nervio. Esto se realiza en el pabellón de operaciones, bajo anestesia general e identificación de los parámetros o coordenadas para la lesión con un equipo de rayos dentro del pabellón. Una de las técnicas, la más usada en nuestro medio, consiste en comprimir este ganglio (llamado ganglio de Gasser) con un balón que se infla sobre él (catéter de Fogarty). El procedimiento en general es muy bien tolerado, con 1 o 2 días de hospitalización en sala, sin mayor dolor ya que se realiza sólo una punción en la mejilla para introducir el catéter. Siempre es posible tener complicaciones, sin embargo en el caso de la cirugía del trigémino por esta vía las complicaciones son poco frecuentes. Otras técnicas van directamente a mover estas arterias que contactan el nervio y producen el dolor, pero son reservadas en general para casos que no han respondido bien a los otros tratamientos. Cuando el procedimiento no ha tenido complicaciones, en general el alivio del dolor es inmediato, pudiendo quedar una disminución de la sensibilidad en ese lado de la cara que en general es bien tolerada por el paciente. En algunos casos es necesario repetir el procedimiento tiempo después de la primera cirugía.

 

¿Qué especialistas deben tratar la neuralgia del trigémino?

Los especialistas en neurología y neurocirugía son los tratantes de esta enfermedad, quedando en general a cargo los neurólogos cuando el tratamiento se resuelve con medicamentos y los neurocirujanos cuando se debe intervenir quirúrgicamente. Los odontólogos también son parte importante en el estudio previo para descartar lesiones dentarias que justifiquen el dolor.
